Liability Release

I give permission to the Program Staff at Liberty Junction Theatre Inc. to arrange any emergency medical care including hospitalization/transportation, if they deem necessary. All participants are responsible for their own medical coverage.

I hereby release the Liberty Junction Theatre Inc. from all liability and claims arising in relation to any matter including personal injury or damage to/loss of property, regarding participation in any activity or otherwise and hereby indemnify the Liberty Junction Theatre Inc. from and against such claims, suits, actions and prosecutions by reason of any activity carried out by myself, the enrolled or my child, whether on or off the premises at which the Theatre Arts Camps are held.

LJT Inc. reserves the right to request withdrawal of an actor from the program, at the company's own discretion, where it is judged that the needs of the particular individual cannot be accommodated without unacceptable disruption to the program. Refund will be issued in these cases for days not attended. Please ensure that LJT Inc. is notified well in advance where medical and other related conditions might present an issue with respect to LJT Inc.'s ability to accommodate a particular actor.

Please note that all of our instructors have had a current criminal reference check.

Photo Release

I give Liberty Junction Theatre Inc. full permission to photograph, videotape, the enrolled/my child, and to publish said photographs, videotapes, film and or/interviews in Liberty Junction Theatre Company publications/printed material, including marketing and promotional materials, and on the Liberty Junction Theatre Inc. official website. I release and forever discharge Liberty Junction Theatre Inc. from all actions, causes of actions, claims and demands with respect to any such use except as agreed in writing.

Withdrawals

If you wish to cancel your registration, requests must be received in writing at least two weeks prior to the first day of the start date. An administration fee of $25 will be charged for each cancellation. After this period, any cancellation requests must be accompanied by a medical certificate in order to receive a refund. You will be refunded for unattended days, less a $25 administration fee.

Liberty Junction Theatre Inc. reserves the right to cancel or re-schedule a camp week or program. 10 days notice will be given and full refund.
